How to make a health, dental and vision claim in Guyana
What to do, in order
- 01Get the right formThere is one form each for medical, dental and vision claims. Download it below, or collect it from your office or the group administrator at your workplace.
- 02Complete the principal's statementThe principal, the plan member, answers every question and writes NA where one does not apply. The patient also signs if over 18. If the patient is an employee, the employer completes the employer's statement.
- 03Have the practitioner complete their sectionThe attending physician, dentist or ophthalmologist fills in the diagnosis, the treatment record and the fees. Ask for it at the visit and take the form home with your receipt.
- 04Attach original receipts and submitOnly original receipts are accepted. Write the total for each expense category where the form asks. Hand the set to your group administrator or to your office; the documents stay with the Society once submitted.

Good to know
Before you start
If the treatment was needed because of an injury, the form asks for the date, the time and how it happened. If the patient is entitled to workmen's compensation or to cover under another medical plan, say so and name the company. The Society may recover from that cover any amount it has paid.
Benefits can be assigned to the practitioner. The application for assignment of benefits on the form lets the Society pay the doctor or dentist directly for a stated period. Charges the plan does not cover remain payable by you.
A photocopy of the signed authorisation on the form is as valid as the original, so keep a copy of everything you submit.

How do I claim on my health plan?
Complete the health, dental or vision claim form, attach your receipts and the physician's statement where the form asks for one, and submit it at your office. We confirm receipt and tell you if anything is missing.
Do I need original receipts for a health claim?
Yes. Only original documents are valid in support of a health, dental or vision claim, and once submitted they stay with the Society. Keep a photocopy of the completed form for your records.
Who completes a group dental or vision claim form?
The principal, the plan member, completes and signs the principal's statement, and the patient signs too if over 18. The employer completes the employer's statement. The attending dentist or ophthalmologist completes the treatment record on the form and gives it back to the patient with a receipt. The group administrator signs for the form when it is received.
