How to make a group and pension claim in Guyana

What to do, in order

  1. 01Start with your group administratorThe person at your workplace who administers the plan holds the group name and number the forms ask for, signs for claims received and sends them to the Society. If you have left that employer, contact the office directly.
  2. 02Group health, dental and visionUse the same claim forms as individual health claims. The employer's statement on the form must be completed, including the last day worked and whether workmen's compensation has been claimed.
  3. 03Group life: death of a memberThe proofs of death are the same as for an individual policy: claimant's statement, physician's statement, identification statement, death certificate and identification. The group administrator supplies the group number and the member's details.
  4. 04Pension benefitsRetirement, leaving-service and death benefits under a pension plan are paid according to the plan's rules. The plan administrator and Corporate Care confirm the options and the documents needed in each case.

Group and pension claims: common questions

How do I claim on my health plan?

Complete the health, dental or vision claim form, attach your receipts and the physician's statement where the form asks for one, and submit it at your office. We confirm receipt and tell you if anything is missing.

Do I need original receipts for a health claim?

Yes. Only original documents are valid in support of a health, dental or vision claim, and once submitted they stay with the Society. Keep a photocopy of the completed form for your records.

Who completes a group dental or vision claim form?

The principal, the plan member, completes and signs the principal's statement, and the patient signs too if over 18. The employer completes the employer's statement. The attending dentist or ophthalmologist completes the treatment record on the form and gives it back to the patient with a receipt. The group administrator signs for the form when it is received.
