How to make a death claim in Guyana
What to do, in order
- 01Tell the Society in writingNotify the office that holds the policy, or the agent, as soon as you can. A short letter or email giving the policy number, the name of the person who died and the date of death opens the claim.
- 02Collect the proofs of deathThe Society issues three statement forms: the claimant's statement, the physician's statement and the identification statement. The claimant's and identification statements are sworn before an officer authorised by law to administer oaths.
- 03Gather the certificatesThe death certificate, the registration of death, the policy document, and identification for both the person who died and the claimant. A police report is needed only if the death was an accident.
- 04Submit at the officeBring the full set to the office that holds the policy. Staff check it against the checklist and give you a receipt. Fees for obtaining the proofs, such as notary fees, are paid by the claimant.

Before you start
The claim is made by the person entitled to receive the money: the named beneficiary, the executor or administrator if the policy is payable to the estate, or the assignee if the policy was assigned. An executor or administrator also supplies the certificate of their appointment. An assignee produces the original assignment.
If there is no named beneficiary, probate of the will or letters of administration, with the schedule of assets and liabilities, must be submitted with the claim.
Where an official inquiry into the cause of death was held, a certified copy of the verdict or findings is submitted with the claimant's statement.
The Society may ask for further information once the documents have been reviewed. Issuing the forms does not by itself confirm that the policy will pay.

Who can make a death claim?

What if the policy has no named beneficiary?
Probate of the will or letters of administration, together with the schedule of assets and liabilities, must be submitted with the claim. The claim is then made by the executor or administrator.
Does the claimant's statement have to be notarised?

Is a police report needed for a death claim?
